Report Submission

1. Nominator must complete the Fairmont Heights High School Athletic Hall of Fame (FHHSAA AHOF) on-line Nomination Form found at https://www.fhhsaainc.com/athletic-hall-fame and submit it electronically no later than midnight, March 6, 2026. All completed nomination forms must be submitted by email to
   athleticHOF@fhhsaainc.com, no exceptions.
2. Nominee must approve being nominated before the committee will process the form. If nominator need more space to support their nominee, they should provide supplemental pages. These pages should have the nominee’s name at the top of each page.
3. Please upload supporting documentation of any accomplishments, such as, All Metropolitan or newspaper article.
4. All information on this form is confidential and will only be shared with the committee members of the FHSAA AHOF

Areas of Consideration
a. Check the category applied
1. Athlete – Person that played the sport
2. Coach – Sponsored Coach of the sport
3. Teacher – Responsible teacher for the sport program, may not be the sponsor
4. Legacy – Student athlete over 50 years since graduation. Outstanding performances in their respective
field, but did not receive any recognition
5. Team/Squad Recognition: Ex: 1959 County Track Champions, 1967 Varsity Cheerleaders, Basketball State
Champions
6. Team Support (Manager, etc.) – Manager of the team, if nominated
7. Team/Squad – Year of the entire squad that is nominated
b. Sports
1. Choose the sport or sports that the nominee is being recommended for, if more than one select all that apply.
They must be an elite athlete.
2. If an approved sport is not on the list, please add the sport.

Definitions

• Elite Athlete is someone who has reached the highest level of performance in a particular sport.
• Legacy Individual is given special consideration pending the following:
  •Minimum 50 years since graduation
  •Outstanding performance in their respective field but does not receive any recognition, i.e., All Conference, Cheerleading, Poms etc.
  • Team/Squad Recognition Entire squad or team receives recognition
  •Ex. 1959 County Track Champions, 1967 Varsity Cheerleaders, Basketball State Champions
  •Other Individual or Company Sponsorship (Donated $1,000 or more to the school)

Accomplishments – Honors

1. Accomplishments must contain information that is verifiable. The information must be clear and precise with pertinent information that describes the nominee’s accomplishments. The athlete must be of elite status in their sport with documented accomplishments, such as, All Metropolitan, All State, Team Championships and etc. All supporting documents must be emailed with the completed nomination form.
2. The captain of the group/team, if nominated should be listed. If he/she is not available, please list the other members of the group/team. This is instrumental in verifying the information.

Post High School Accomplishments:

1. Accomplishments after graduation from FHHS pertaining to your sport or any achievements that will assist with the committee’s decision to select your nominee being a hall of Famer. Write as much supporting information that is verifiable in this section. Be very detailed and precise.

Other Accomplishments/Achievements:

1. This section is for professional /accomplishments/achievements that FHHS was instrumental in shaping the nominee or whatever you feel is noteworthy for the committee to know and understand about the nominee that will help them to stand out.
