NOMINATION FORM

Nominees in all sports categories must be a FHHS graduate, ten years removed from high school, played a varsity sport for at least two years, achieved All conference status or better. Nominees in the coaches category must be 5 years removed from FHHS. All nominees are ENCOURAGED TO BECOME A PAID MEMBER OF THE FHHS ALUMNI ASSOCIATION prior to their HOF Induction at https://fhhsaainc.com/membership-info/.

Minimum 50 years since graduation
Outstanding performance in their respective field, but does not receive any outside recognition (example) All Star selection, Cheerleading, Poms etc.

* Team/Squad (Year) Entire squad or team receives recognition (i.e., 1959 County Track Champions, 1967 Varsity Cheerleaders, Basketball State Champions)

Accomplishments/Honors (i.e., Cheerleader Varsity 3 years; Track won 4×100 County Champion, All Conference, All Metropolitan, All State selection: Team Recognition (County, Regional, State Champion)
